I accept most major insurance plans, such as Cigna, Blue Cross and Blue Shield, United ,  Optum, Aetna, including some EAP's.
 

There are some services I provide that insurance does not cover.
 

If I am not on your insurance plan, a self-pay plan can be arranged. 

With your permission, before we meet I will contact your insurance provider to verify your benefits, find out your co-pay, and let you know up front how much to expect to pay on your first visit.
